- Company-Offered Plan Coverage
- If you are covered under our company-offered insurance plan, you will receive a discount of 40% of the patient portion of the costs, including deductibles, co-pays or co-insurance.
- Other Insurance Coverage
- For team members covered by insurance plans from other providers, the discount will be evaluated on a case-by-case basis.
How do I qualify for the discount?
To ensure eligibility for the team member discount on Innovate’s outpatient services, please note the following requirements:
- Financially Responsible Party
- The active team member seeking the discount must be listed as the Financially Responsible Party for the therapy services. This ensures that the discount is applied correctly.
- Automatic Payment Options
- To streamline the payment process and ensure timely transactions, the team member must agree to have the balance paid automatically on a monthly basis. There are two options available for automatic payment:
- Auto Pay with a Credit Card or Checking Account: Payments can be set up to be automatically deducted monthly from a credit card or checking account on file with Innovate Physical Therapy.
- Payroll Deduction Process: Payments can be deducted directly from the team member’s paycheck.
